Case Follow-up

• Biopsy of right ischium was surgically done and submitted for culture, cytology, and histopathology.

• Culture: Negative

• Cytology: Pleomorphic spindle cells were seen that may suggest neoplasia or a reactive process. Osteoblasts, a few small lymphocytes, occasional neutrophils, and rare osteoblasts were also noted.

• Histopathology: Chronic osteomyelitis with osteonecrosis, osteolysis, and skeletal muscle myodegeneration and mild myositis.

• Treatment: Six-week course of antibiotics.

• Future plan: Repeat physical exam and radiographs to note improvement or need for alternative treatment.
